WAIVERS

Available for Download

LDT WAIVER

Please fill out and bring waiver with you to class. Waiver is mandatory for all classes with live fire. Need to be completed once a year.

SOUTH RIVER WAIVER

If you are taking a class at this location please fill out and bring the waiver with you to class.
